I have just picked up "Concepts, Techniques, and Models of Computer
Programming" by van Rot and Haridi. It's quite a big thing (900
pages) but from a brief perusal and look at the TOC, seems to cover in
depth quite a range of programming techniques including higher-order
programming in the declarative programming techniques chapter.
