[Ann] tolua++ 1.0.93

Hi.

I'm releasing tolua++1.0.93. Mostly bugfixes that accumulated on the
SVN over the years.

- Build process bootstraps.
- Added hooks to handle custom types.
- Added hooks to handle exceptions.
- Fixed issues with newer GCC compilers.
- Changed to MIT license. See COPYRIGHT file for details.

The main reason for the release is the license change, to the MIT
license (same as Lua).
